Due to operational reasons we're postponing the opening of our new Community Place service until the new year. 

We’ve been working closely with Calderdale Council who manage Community Place, and Calderdale GP Alliance to set up a service for those who have had a hospital stay with us, and are medically fit though need some social support before returning home.

Once it opens, a multi-disciplinary team will be on site and medical support will be given by local GPs. The aim will be to encourage independence through rehabilitation and support.

Community Place is not aimed at those people who require medical care or 24 hour nursing and is intended for short stays of a few days.
