Finalists:

• The Calderdale Community Specialist Palliative Care Team and the Out of Hours Palliative Care Nursing Team-Partnership working between the Calderdale Community Specialist Palliative Care Team and the Out of Hours Palliative Care Nursing Team, Overgate Hospice and Marie Curie, to improve access to specialist palliative care support for patients and families in Calderdale.

• Food for Life Partnership-Working in partnership with other agencies to promote a new hospital model focussed on patient food and nutrition leading to an improvement in patient satisfaction, reduction in patient food waste and a proposal for improved vending facilities.

• The Vascular Surgery MDT "Grand Round"-Introducing a weekly, multi-disciplinary ward round, or "grand round", of vascular surgery inpatients to take the expertise of various people directly to the patients’ bedsides, creating a pleasant, productive atmosphere, with the aim of improving patient safety, experience and outcomes.
