Finalists:

• Ali Tooley - Redesigning the cancer pathway for patients diagnosed with a new lung cancer, giving patients direct access to a specialist nurse from the very beginning of their journey, and reducing diagnosis times and visits to hospital.

• Lung Cancer team-Introduction of a 2 week programme for patients with patellofemoral pain and of video technology for patients to undertake their rehabilitation at home to improve their outcomes and reduce the need for 1:1 physio sessions.

• Post Natal Buggy Walks- Introduction of post natal "buggy walks" to address the needs of new mothers in Rastrick: to protect them from the risk of social isolation and postnatal depression by providing an opportunity to be physically active with other new mothers in a positive and supportive environment.
