Having a stroke can be a life changing and traumatic event, which often takes time and the input of a whole multi-disciplinary team to help people back to normal life.
The way this rehabilitation is delivered in the Trust is being transformed on Ward 7b at CRH.
The improvement work is a theme within the SAFER programme, and seeks to build on the already strong multidisciplinary links, with different professions working together more closely and sharing expertise to help patients achieve agreed rehabilitation goals.
Central to this are morning huddles where crucial information is shared and plans reviewed for each patient.
The team, led by Consultant Stroke Physician, Dr Anand Nair have also produced a detailed transformation plan, covering all aspects of how the ward operates. This means when the work is complete, the service will be second to none, with an aim to improve outcomes within a shorter inpatient stay and linking effectively to community services.
