A new Primary Care Discharge Co-ordinator Service has been created which aims to improve patient experience and reduce admissions to hospital.

The team, which is based within the primary care setting across Greater Huddersfield, comprises senior nurses who will be working closely with GPs and a range of services to support patients to remain in their own homes.

The team will mainly be involved in the care of patients aged 75 and over who do not currently have any other community healthcare service involvement.

Lead nurse, Gill Runkee, said: “We are hoping it is going to be very much tailored to what the patient wants and be fully patient-centred. We aim to be able to keep them where theywant to be - and that is in their own home. This will have quite a big impact on their quality of life and how they manage their condition themselves to a degree."
