There's fantastic news for the CHFT stroke team!
 

After a great deal of hard work from all members of the multidisciplinary team the stroke team have managed to improve their SSNAP score (Sentinel Stroke National Audit Programme) from a D to a B; which puts us second in the Yorks and Humber region after Scunthorpe General Hospital.

The turnaround has been due to introducing new ways of working to make the service better for our patients provided by tremendous team spirit on the unit.

These changes in practice include:

•  Making the thrombolysis process slicker. Stroke team attends the patient in A&E as soon as the patient with a suspected stroke arrives and they are the first to see the patient
• CT scans are guaranteed  within an hour after arrival during daytime
• Rehab teams including physios and occupational therapists are working over the weekend so if a patient is admitted on a Friday they are seen on Saturday and their discharges are not delayed
• Better data collection and audit compliance
• Better information for  patients and their family including a Joint Care Plan on what kind of a stroke has been diagnosed, what the function is and aims to strive for.
• Improvements in integrated care pathway with improvement in data capture and reflecting the changes in practice
• Stroke Consultant on call has been seeing the patients within short time during working hours.

Stroke Clinical Lead Dr Pratap Rana said: “Our nurses are working round the clock delivering thrombolysis and are under immense pressure and cope very well. This is much better, streamlined service for our patients and their families and we are delighted to see this recognised in our new care rating in the National Audit.”
