We've had some good feedback about the changes to Datix that happened on 16 May. There a few things we'd like to update colleagues on to make sure our incidents are reported accurately.

Reporting incidents and near misses is all about learning what could have been done differently (if anything), identifying as well as providing an opportunity to review the processes to reduce patient harm incidents. 

The attached briefing outlines the key updates/feedback including:

• New feature welcomed by colleagues who report incidents
• Coding an incident – call us if you need help rather than selecting OTHER
• Completing the investigation section is important as we report our incidents externally
