Thanks to everyone who has been in touch with a view to becoming one of our new GP Locality links  - we will be in touch soon.

However we still have places on the scheme and colleagues from any role  (clinical or non-clinical) can apply. Closer ties with our GP colleagues will enable our Trust to better understand the care and services both in our hospitals and in communities which our patients and their families need.

You would become the main point of contact here for the PCN and liaise with them to share good news, such as new service developments here at CHFT and also be a point of contact if they have anything to raise with us.

**Primary Care Networks (PCNs) are an exciting new venture, where doctors and other healthcare professional such as nurses, physiotherapists, pharmacists and the voluntary sector work collaboratively to provide excellence in health care to their collective practice populations. This new network approach will see more integration of GP, community and hospital colleagues, who will be working together to develop new pathways of care and workforce models that are fit for the future.
