Another fab leap digitally as our local GPs are now seeing in real time what is happening to their patients at CHFT.

It's called Community View - where EPR and SystmOne link up - and it's already having a huge impact with Dr Dinesh Kumar, GP at Plane Trees Group Practice in Halifax.

Dr Kumar, told us: "It is very convenient to be able to access real-time information and saves really valuable time in direct patient care. 

"Recently one of my patients was admitted to Calderdale Royal over the weekend. The patient attended the practice on Monday morning and I was able to access and check what investigations had been carried out. It was more efficient, safe and I was able to see the results and plan in the consultation right there and then.

"Another patient was admitted to a different hospital without  access to electronic records by Primary care which meant spending a lot of time chasing the information.  

"Primary care in Calderdale values the link as vital, and I know all GPs  feel the same".

With Community View we can see the medications the patients are on in the community, as well as a raft of other information from their primary care record. It also gives GPs access to our EPR so they can see in real time too what’s happening to their patients, as well as accessing results and seeing planned appointments.

Currently this is limited to practices in Calderdale and Greater Huddersfield.

Community View should:

  • Help with informed clinical decisions
  • Improve Meds & allergy history
  • Allow GPs to access discharge summaries
  • Allow primary care to view future appointments, removing unnecessary admin and duplication
  • Minimise gaps in the patient history
  • Reduce referrals and admissions
  • Improve patient experience & outcomes, with patients becoming more confident in Healthcare establishments.
  • Help on the path to a shared record.

Our Consultant and Chief Clinical Information Officer, Graham Walsh, said: "I'm really pleased to share this news of yet more improvements, which we're hopeful will bring many benefits to the way we care for our patients.

"It's been a real team effort as the digital team have been working closely with partners in primary care to help us all get the best out of Community View.

"And this is only the start! We are really close to adding social care and community records to this shared view, which will only enrich the experience further. It will also eventually help us to access patient records from other healthcare providers significantly improving patient safety".
