CHFT is looking for volunteers to strengthen our links with GP colleagues across our area.

Primary Care Networks (PCNs) are an exciting new venture, where doctors and other healthcare professional such as nurses, physiotherapists, pharmacists and the voluntary sector work collaboratively to provide excellence in health care to their collective practice populations.

This new network approach will see more integration of GP, community and hospital colleagues, who will be working together to develop new pathways of care and workforce models that are fit for the future.

Would you like to be part of the new world?

We need 10 colleagues – and they can be in any role – to be our points of contact with the new GP localities (PCNs) across Calderdale and Greater Huddersfield (five in each).

Closer ties with our GP colleagues will enable our Trust to better understand the care and services both in our hospitals and in communities which our patients and their families need.

You would become the main point of contact here for the PCN and liaise with them to share good news, such as new service developments here at CHFT and also be a point of contact if they have anything to raise with us.

So, if you’re interested, or want to know more, please email Lisa Williams. Lisa.williams@cht.nhs.uk

Tell her:

  • Your name & contact details
  • Your role
  • And briefly sum up why you’d like to be a CHFT Locality Link.

Most networks are geographically based, so you may want to be part of the team delivering care to you and your family.
