Calderdale’s Enhanced Reablement Team is a new branch of the existing Reablement Service in Calderdale, formed from a partnership between CHFT and Calderdale Council.

They started as a pilot scheme in July 2018, running for a year initially and is already getting excellent reviews from our patients and their families.

They are a team of CHFT Specialist Occupational Therapists and Specialist Physiotherapists from the Support and Independence Team and work with trained Council support workers from the established Reablement Care Team.

The aim of the service is to facilitate early/ timely discharges of patients from the acute hospital wards.They cover the whole of Calderdale and accept admitted patients from all inpatient wards who have both rehabilitation and care needs. Their aim is to reduce the number of hospital bed days using a discharge to assess model.

The Enhanced Reablement service enables people to return to their own home with a short period of support and structured, individualised functional rehabilitation from day of discharge.

Outcomes have been very positive so far and show:

  • a reduction in patient dependency of up to 25% meaning ongoing care needs are less,
  • up to 50% reduction in hospital length of stay, freeing up beds more quickly
  • good qualitative feedback from patients via Friends and Family Test.
