After a recent incident, Sister Sally Roberts from ward 6 (pictured middle) and her team have been working closely with the tissue viability team to devise an action plan to improve practice.

Sally contacted the Quality Team to request a bite-sized learning on the topic, which they were pleased to do.

The learning - Hospital Acquired Category 3 Pressure ulcers

  • Have patients had a waterlow risk assessment done within 6 hrs of hospital admission?
  • Have “at risk” patients had a skin inspection, skin bundle, nurse rounding and pressure ulcer prevention care plan initiated?
  • Is the frequency of skin bundles and nurse roundings specified in the pressure ulcer prevention care plan?

Specific, Individualised care planning, interventions, evaluations, and documentation are all key to safe & effective pressure ulcer prevention.
