This year CHFT  has launched a programme to transform our outpatient services and we've presented our work so far to NHSI.

Calderdale and Huddersfield Healthwatch found that many patients expressed would like to access these services in a different way including by phone, video links or outside of the hospital setting and closer to home.

Project Manager, Sharon Appleby, said: “ Our OP system is now very outdated  and hasn’t changed for years with  many repeat visits for many of our patients involving travel into our hospitals. We know by offering other choices to them they can select how and where their appointments  are held at times best for them” 

Project 20-20 is in partnership with our commissioners, patients reps, GPs and CHFT staff. Our ambition is to transform 20 clinical services by 2020 (hence the 20-20 logo).

Several services have already been transformed to improve both our patients experiences and how they access our services. Short videos has been created to demonstrate the some  of the great work our clinical colleagues are doing and shows examples from Cardiology, Colorectal and Yorkshire Fertility Clinics. Watch here.

The project generated a lot of energy and interest at the annual planning event with some new ideas being taken forward with speciality teams. We are now encouraging all service teams to work together to redesign outpatients services and we would love to hear from you!
