Our Quality Team has discovere there are errors when recording pressure ulcer incidents for newly-admitted patients on Datix.
As many as 50% are recorded wrongly so it is important we tackle this.
Here our Clinical Governance Support Manager, Lynne Taylor, guides colleagues on the right way to record them.
Lynne, said: "It's important when patients are admitted with pressure ulcers we record the incident as a matter of course as part of their patient records and to ensure safe care. However, as the patient came into us with the ulcers, we didn't in fact cause the harm".
So when completing incidents for patients admitted with pressure ulcers on Datix please:
- Do not select CHFT as the organisation. You MUST select the correct organisation from the drop-down box
- When choosing the level of harm please select NO HARM
- And finally you should select GREEN when recording the severity
