From next Monday,  patients attending outpatient clinics will be able to register to access their records online through a patient portal called Your EPR. 

In preparation for this, and to help us save costs, we have stopped sending patients a paper copy of the GP letter following their outpatient attendance, which we used to do for some patients.

Your EPR lets patients view the following information about their health care: 

  • See GP copy letters (written after 4th June 2018)
  • Review recorded allergies
  • See future appointments
  • See selected data from hospital records including test results
  • View patient education leaflets

Our patient leaflet (see below) is available in outpatient clincs and explains more and how to register, including for those patients under the age of 16. 

Will this change current processes?

Patients will be required to register in person when they attend their Outpatient appointment by signing a consent form. Their account will then be created and they will be sent an email to complete the registration.

There will be no change to how letters are dictated.  Medical Secretaries should follow the same process as they do now when selecting letter templates.  The consent they should look for is called ‘YOUREPR’ and can be found on the top right-hand banner in power chart.  This will see an end to searching for consent within the system.
