Patients taking warfarin need follow-up INR checks after discharge.
Recently we have had a number of incidents where patients' discharges could have been better.
So here is the low-down on contacting the anticoagulant team.
Bite Sized Learning: INR levels
We need prior notice of discharges, especially for those patients who need a home visits.
All patients should have a follow-up appointment arranged before discharge by ward staff. Please do not ask patients to contact us directly.
The anticoagulant team can help with dosing advice and arranging appointments.
Opening hours: Monday to Friday 08.30 to 17.00 (closed on Bank Holidays.)

If discharging at weekends: Email the office with patient NHS number and details of warfarin dose and when next INR is required.
