We’re planning a Primary and Secondary care initiative mid-June to encourage patients to bring in all their medicines.

(including all medicines that have been bought over the counter as well as those that have been prescribed) and diabetes related equipment (blood sugar meter, test strips insulin, pen etc) into hospital with them should they be admitted.  This is in order for us to:

• Check all medicines that patients are taking
• Plan effective care
• Encourage self-management where possible
• Ensure doses of medicines are not missed

We will be sending posters out for display throughout CHFT premises.  We will also be sending the posters out to health centres, ambulance service, libraries, pharmacies, schools with a covering letter from Rob Moisey and Mike Culshaw .
