We’ve seen an increase in the amount of confidential information (colleague and/or patient names) being included in the wrong area of Datix – so our learning this week is aimed at improving that. Our Datix incident reporting information is seen by staff in the Trust, but the description field is uploaded to a national database, where it is visible to those in NHS Improvement.

To avoid a breach of data protection regulations it's important colleagues never include patient or colleague identifiable information into the Summary of the incident/near miss box.
