Arriving on wards soon, the second of our themed learning publications - Learning from Death - reports back on all the work we're doing at CHFT to improve the care pathways for our patients at the end of their lives. 

In this newsletter (attached) you will learn more about our mortality ratios and what we have been doing to achieve these improvements. You will read more about our learning from deaths progress and specifically what we have learned from mortality reviews. One of the major changes is a new umbrella approach to learning from deaths which aims to align all the strands to continue to improving patient’s outcomes and experience. 

The idea of the newsletter is to discuss key themes of learning colleagues and then display the inside cover as a poster in staff areas - part of our new approach to learning, along with our bite sized learning articles each week.

And following feedback from  Sisters, the bite -sized learning editions will be collated and shared regularly on paper so that colleagues have the opportunity to read them if they've not had chance to see them in CHFT Weekly.
