Not completed your quick and easy FFT survey yet? Worry not - you can also do it on your smartphone by scanning the QR code.Just launch your QR reader app and scan the code. You'll need to use your payroll number (known as an assignment number) as your user name and password.

The survey closes on Sunday 3 September, and has been sent to all colleagues with a CHFT email address. You'll receive weekly reminders from the Staff Friends and Family email address each week unless you've completed it. 

Responses are strictly confidential and nobody from CHFT will see any completed questionnaires, or be able to identify individual responses. Picker (who manage the survey) will analyse the results independently and no individuals, or their responses, will be identified.

Even if you completed the last survey - please take the time to complete it. The questions will be:

• How likely are you to recommend Calderdale and Huddersfield NHS Foundation Trust to friends and family if they needed care or treatment?
• How likely are you to recommend Calderdale and Huddersfield NHS Foundation Trust to friends and family as a place to work?
