We have a new yellow bags scheme to ensure patients leaving us for care homes keep all their information together to provide better, seamless transfers and care.

Julie Williams, Locala's  Macmillan Nurse Specialist for Palliative Care Education in Care Homes, spent time last week in wards and departments at Calderdale Royal and HRI spreading the word about this initiative. She said everyone she spoke to about the bags was really enthusiastic and looking forward to using them.
