On Monday the pharmacy stores will be centralised to a main store on the HRI site and there will be a new online form for ordering additional stock. 

 

There will be no change to the pharmacy stock top-up service to wards or clinics. Orders will be raised by pharmacy stores as usual, assembled at HRI and delivered to CRH for distribution.

 

We will continue to supply a few items from CRH stores: IV fluids, enteral feeds, some drugs which we supply to clinics e.g. ranibizumab, aflibercept and Botulinum toxin Injection.  Individual clinics will be informed.

 

How you can help us

We’ll continue to provide the same service to colleagues as normal as we move (over the weekend), but ask that you order stock medicines as early as possible as we have to obtain these from HRI.

 

Extra wards stock/ad-hoc requisitions – new online form from 27 March

An electronic form is available via the intranet which is sent directly to pharmacy stores. After Monday 27 March paper stock requisitions will only be accepted for IV fluids and enteral feeds.

Most items will need to be supplied from HRI – so ensure that you order stock items in good time.

Requisition forms for IV fluids and enteral feeds

These are available on the document repository – completed paper forms should be put in the post box outside CRH pharmacy store or dropped off at HRI pharmacy store.  These will be processed as soon as possible.

 

Calderdale Community Clinics and Health centres

Please continue to return/send your ward stock order to CRH Pharmacy Store in your stock box or via internal post.  Transport will collect from CRH on a Monday as normal and deliver.

 

Out-of-hours additional stock

Please check the following before contacting the on-call Pharmacist:

  • Has your top-up stock been unpacked and put away?
  • Is the item kept in the emergency drug cupboard? (Site Coordinator can assist)
  • Can you borrow the item from another ward? (Ward stock lists are available on the intranet)

 

We believe these changes will have a significant benefit to wards – though we do need to make sure that you check stocks on the ward before sending requisitions to pharmacy and that you allow enough time to obtain stock from HRI.
