Familial Hypercholesterolaemia Band 7 Nurse
Fixed term 2 Years Secondment opportunities welcomed

The British Heart Foundation (BHF) in partnership with the Vale of York Clinical Commissioning Group, and the Yorkshire and Humber Strategic Clinical Networks, is supporting the establishment of a region-wide cascade screening service for Familial Hypercholesterolaemia.

This exciting new programme, will, when fully operational be the largest such programme in England and has the potential to offer significant health benefits to patients and their families in the Yorkshire and Humber region. York NHS Foundation Trust is acting as host for this regional service, which will cover York, Leeds, Hull and Calderdale. The post offered will be primarily based in the Huddersfield and Calderdale NHS Trust.

In particular we would like to encourage applicants from a range of healthcare provider settings (i.e. acute hospitals, district general hospitals, community and primary care settings) so that the team’s skills and clinical experience reflects the patient pathway.

The aim of this cascade genetic screening programme is to improve lives and patient health outcomes through early identification of patients and their family members affected by Familial Hypercholesterolaemia and the implementation of early targeted interventions.

Successful candidates will be expected to work as part of the team led by the Project Manager and will have specific responsibility for development and delivery of the service in a defined regional geographical patch. The post-holder will provide important administrative and/or clinical skills to the service and establish close collaborative working between the BHF, the host organisation, genetic diagnostic laboratory services, local GP practices and CCGs.

The post-holder will collaborate with local GP’s, hospitals and local lipid clinics, across the region to develop, set up and deliver the service. The post-holder will be able to communicate complex information to a wide range of staff groups, i.e. Consultants, multi-disciplinary team members, internal and external stakeholders. The successful candidate will benefit from being included in the national network of British Heart Foundation Healthcare Professional Alliance members with training and network opportunities included.

Successful candidates will be required to travel across the region to sites participating in this programme.

Please note that some of our substantive vacancies may be available on a smaller number of hours than advertised.  If you would be interested in applying on this basis, we would encourage you to contact the appointing manager to enquire further.
