As well as the care delivered in hospitals our Trust cares in the community for our patients of all ages in Calderdale.
Here matron Jan Ghee tells more about the team whose door "is never closed"!
1 How many patient visits do our community teams make a year? Over 600,000
2 Can you tell us which teams are based where and what are the limits of the area we cover?
The teams are based in different buildings across the community such as Todmorden Health Centre, Brighouse Health Centre, Broad Street Plaza, Beechwood Health Centre, St John’s Health Centre, Nursery Lane Health Centre, Children’s Centres and GP practices. They cover the geographical area of Calderdale and some of teams cover a wider area such as children’s therapies who cover South Kirklees and the Lymphodema team who cover Huddersfield and Wakefield.

4 What makes you proud about our community teams?
Their resilience: our ‘doors’ (even though we don’t actually have doors!) are never closed; they care for all those in need and keep going at times of great demand with a smile and compassion even when affected by issues beyond their control such as bad weather, road closure and at the beginning of last year the floods.
