From Monday 14th October, Yorkshire Ambulance Service (YAS) need to be made aware when a patient is receiving barrier nursing care or isolated care by completing this online form.

This is a confirmation of infection prevention and control arrangements form, and will ensure YAS continue to provide isolated care during the patient's transport journey. A clinician involved in the patient's care must complete the form.

If the form is not received and approved by YAS prior to the journey, the patient will travel with others. Forms will be rejected where evidence of barrier nursing or isolated care has not been provided.
