Delirium is a serious medical condition that can lead to poor outcomes, and is preventable and treatable, but early detection is paramount. 

The completion of the delirium screening tool has now moved to a nursing task, although doctors and allied health professionals can however also complete it.

All patients over the age of 65 must have a delirium screening assessment completed on EPR. It must also be completed on any patients displaying signs of delirium such as new and increased confusion or decreased arousal. The delirium screening tool should also be completed on patients who are identified as at risk of developing delirium.
