Our Ageing Well Team; Jackie Wilkinson, Michelle Sykes, Rachel Schofield, Faye Batley and Mike Stamp have been shortlisted alongside partners across Calderdale in two categories at the HSJ Awards in November.

Ageing Well is a multi-professional team of nurses, an Occupational Therapist and a Physiotherapist – known as Ageing Well Practitioners (AWPs). They work closely with the GP Alliance, voluntary and social care sectors across Calderdale as part of an Integrated Neighbourhood Team, proactively identifying people over the age of 65 who may have a mild to moderate risk of frailty.

Their aim is to meet and provide a holistic assessment to identify and promote quality of life for extended years. Each AWP has built relationships with support outside of the traditional GP or hospital service and has a deep understanding of the Health and Social Care offers locally. Their in-depth knowledge of the service means they can also refer into specialties quickly, often avoiding the need for the patient to visit their GP or hospital.
