A new Community Healthcare Division intranet page has been launched today, to promote the division’s services and act as a valuable resource for our community colleagues, as well as wider colleagues across CHFT. Having all the information wrapped up together in one place makes access for colleagues across CHFT much easier.
The information has been written, collated and added by Community Practice Educator and Professional Nurse Advocate, Rachel Jackson, and Senior Clinical Educator for AHPs, Arley Byrne.
Arley said: “People often think the Community Division solely cares for patients outside of the main hospital sites, but our AHP roles within the division (Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, Podiatry and Orthotics) have a massive footprint across our inpatient settings.
“There is information about the Urgent Community Response Team, Home First and Virtual Ward, Nursing services and Allied Health Professions. There is also a page for Community staff to access resources and training relevant to their roles.
“Colleagues have quick access to information about what we offer and how to make a referral into the services. Need something from us in the Community Division? It's all there on our page!
“Our mission is to ensure patients remain safe in their own homes and avoid hospital admissions. We aim to be as proactive as possible, and the more you are aware of our presence and offer signposting, the better we can support our community. Happy clicking!”
