New statutory entitlement to unpaid carer’s leave came into force, under the Carer’s Leave Regulations 2024 with effect from 6 April 2024.
Updated in line with Flexible Working Bill statutory change. As of 6 April 2024 employees can make a statutory request to make permanent changes to their contract from their first day of employment.
Transition Policy (Transfer of Care from Children’s to Adult Services)
- Added CHFT to use ‘You’re Welcome’ criteria to benchmark and produce action plan as required;
- Added role of Transition CNS for neurodisability and responsibility
- Added role of Children’s Board and responsibility
- Changed job title of learning disability role from Matron to Consultant Nurse
- Acknowledge the other National transition tools and bespoke models in use in speciality areas
- Added LD flag, ensuring GP offers annual health check
- Section on psychological needs of young people added
- Updated safeguarding and LAC sections
- Updated resources
Full policy reviewed and re-written to align with the Patient Safety Incident Response Framework.
