As part of the new Patient Safety Incident Response Framework (PSIRF), colleagues carry out After Action Reviews, Thematic Reviews and Duty of Candour.

PSIRF has replaced Serious Incident (SI) reporting, to allow us to be more flexible, transparent and compassionate to learning responses and investigations, focusing on understanding the different factors that contributed to the incident and ensuring that we all learn from them.
