CHFT is taking part in a national End-of-Life survey to improve care at the end of patients' lives.

The survey will gather feedback from colleagues who provide care for end-of-life patients in hospital. Your feedback will support ongoing care improvement within the Trust.

If you look after patients receiving end-of-life care – and no matter what role you have - please complete this short survey. You have until Sunday, 30th June to participate.
