We've recently welcomed Calderdale and Kirklees Carers Count to help support the launch of the West Yorkshire Carers Hospital Discharge Toolkit.
Carers Count representatives, Shazia Bi and Louise Spencer, support unpaid adult carers living in Kirklees. Their colleagues, Lydia Woodall and Beth Sykes support the Calderdale region.
They met with various AHP colleagues at CRH and HRI to talk through the benefits of the new toolkit and to inform our colleagues about the services that support unpaid carers who use our hospitals. Senior Clinical Educator for AHPs, Arley Byrne, said: “The team and all those involved with Carers Count are doing a fantastic job for our carers and ultimately our patients across Calderdale and Kirklees.
“It was great for some of our Allied Health Professional services across both sites to connect with Carers Count to further strengthen the patient journey through hospital and back into the community.
“The next steps are to link up with our Nursing and Community colleagues - so keep your eyes peeled.”
Carers Count offer training in person or online about the toolkit. Email them on infor@carerscount.org.uk to request this. Colleagues can also help by identifying unpaid carers and signposting them to their local carer on (in) your hospitals, community services and social care.
