The See Who I Am care plan is a person-centred document that any colleague can complete with the patient or with the support from family or carers.

It identifies essential information about patients, such as their individual preferences, characteristics, support needs, likes and dislikes, routine at home and life history.

It is kept by the bedside and guides person-centred care planning and individualised care.
