Our new Patient Safety Incident Response Group Policy supports the requirements of The Patient Safety Incident Response Framework (PSIRF) and sets out our approach to developing and maintaining effective systems and processes for responding to patient safety events and issues for the purpose of learning and improving patient safety.

It outlines the process for identifying, reporting, managing, investigating, and learning from patient safety events (formerly described as patient safety incidents), including Serious Incidents (SIs), Never Events, and incidents/events which are notifiable under the Trust’s Duty of Candour.

Alongside the new Policy, a Patient Safety Incident Response Plan (PSIRP) has been written to describe the activities that we have undertaken to prepare to transition over to PSIRF. 

The PSIRP sets out how CHFT will respond to patient safety incidents reported by service-to-service referrals, our staff, patients, their families, and carers as part of the work to continually improve the quality and safety of the care we provide.  The plan will remain flexible and consider the specific circumstances in which patient safety issues and incidents occurred and the needs of those affected.

It outlines our top 10 local patient safety incident priorities - which were agreed following a review by the Quality and Safety Team with support and involvement from the Divisional Leadership Teams, the Safeguarding Team, Mental Health Team, Workforce and Organisational Development and other stakeholders.
