As part of our work to introduce the new Learning from Patient Safety Events (LFPSE) system, Datix needs to be updated.
This work will happen this coming Thursday, 29th February – and for one day only between 09:00am to 20:00, colleagues should log incidents using the link below.
The new LFPSE is an NHS England directive and applies to all NHS Trusts within the UK.
Whilst the system is unavailable, please use the link below to access a manual incident reporting form.

The Incident and Risk Team will advise when the system is back up and available to use.
What are the main changes I’ll see when I access Datix after the changes have been made?
The main changes to note when submitting an incident (patient safety event) on Datix, are the extra questions that will require an answer. These have been set by NHS England. Here are a few examples:
- Which things were involved in what went wrong?
- At what point was the event (incident/outcome) detected?
- If Responsible Specialty not found, please specify.
- Does the incident appear to relate to any of these known safety challenges?
When you submit your incident on the new system, the form will automatically be shared with NHS England.
