Meet Clinical Nurse Specialist, Anisa Jabeen.  She'll be providing dedicated support to palliative patients in Calderdale care homes, following a successful innovation bid for a new post within the Community Specialist Palliative Care Team. 

Her role is to prevent avoidable hospital admissions, support care home staff with early recognition of deteriorating patients, prompt symptom management interventions, facilitate Advance Care Planning discussions and ReSPECT decisions, and support the patient’s family so that patients remain in their chosen place of care until end-of-life. 

Anisa began her two-year post in July and, linking with the QUEST* team, has already made a positive impact on patient care by improving symptom management for those patients in last days of life and improving patient safety by reporting medication incidents.   

She said: “Ensuring patients in care homes are receiving the highest standard of care in their months, weeks and days of life is a privilege. I am building excellent professional relationships with care home staff - with a multi-disciplinary approach we hope to standardise care throughout all care homes in Calderdale”

Update on palliative care services at CHFT

The Hospital Palliative Care team and the End-of-Life team/Bereavement Service now sit together with the Community Palliative care team under the leadership of Abbie Thompson, in her new role as Nurse Consultant for Palliative Care in the Community Healthcare division. Many congratulations to Abbie and best wishes for her new role.

The hospital team and the End of Life/Bereavement service will continue to be located in the hospital.

Both the hospital and community teams will work closely together to ensure an integrated approach to deliver individualised, compassionate, person-centred care for all our end-of-life care patients and their families.
