The Yorkshire and Humber Care Record (YHCR) is a digital shared care record solution across West Yorkshire which enables patient information from multiple provider services (general practice, hospital trusts, community healthcare, social services, and mental health teams) to be accessed securely on one platform.

By linking information systems, the YHCR supports appropriate health and care staff to access health and care information in real-time, at the point of care, whilst also allowing patients to be more actively involved in their own health and wellbeing. This is a significant step forward for both patient safety and informed decision making.

Airedale NHS Hospital Trust and Calderdale and Huddersfield NHS Foundation Trust have provided key maternity information into the YHCR, including delivery summaries, maternity booking forms, mothers discharge summaries, pregnancy outcomes, pregnancy conditions and maternity care plans.

Bradford Teaching Hospitals NHS Foundation Trust has also provided patient data into the YHCR, including information regarding clinical appointments, demographics, discharge summaries, accident and emergency discharges and radiology reports.

In September 2022, Leeds Teaching Hospitals NHS Trust became the first WYAAT trust to go live with consuming patient data from the YHCR, allowing LTHT staff and Leeds Care Record users to view regional patient data via the Regional Tab in PPM+, or in the Leeds Care Record.

This month, Mid Yorkshire Teaching NHS Trust went live, providing valuable patient data including patient demographics, inpatient appointments, encounters, accident and emergency visits and documents such as discharge summaries, clinical letters, and maternity documents into the YHCR. Having access to this patient information will improve the provision of care delivered to patients across West Yorkshire.
