Our Children and Young People team recently held a cross-divisional event for the transition of young people from child to adult services.

Transition is the purposeful, planned movement of adolescent and young people with long-term conditions, from child-centred to adult orientated health care systems.

The event featured CHFT's Young Person's Ambassador, Drew Greenwood, describing his experiences as well as his advice for colleagues.

Lead Nurse for Children and Young People, Julie Mellor, said: "You could hear a pin drop in the room as he spoke"

Drew told the group: "Patient-centred care is so important. We need to ensure that we plan early and adopt a one-size-does-not-fit-all approach to avoid other young people falling off a cliff, or hitting a roadblock as they move from child to adult services."

So, if your role involves the care of young people between the ages of 13 to 25, the team are asking you to #thinktransition.
