We want to understand colleague awareness around incident reporting and patient safety. How is learning from incidents shared and improvements made in your area? Does your area excel in learning from incidents? Could things be a bit better?

To help us understand where extra support or training might be needed, our Quality and Safety Team is asking colleagues to complete a quick survey.

There are questions on staffing levels in your ward/area, as well as finding out how suggestions and learning is communicated, and actions put in place.

We all have a part to play in patient safety - whether we're clinical or not, so the survey is open to all. There is a comments box at the end of the survey for further comments.
