A new file naming convention has been introduced for saving documents and images onto InPhase. This new standardised approach will make it easier to find the most recent version of documents, reducing the chance of errors and miscommunication.

When uploading all documents, images, emails, or other files to InPhase, please first save the document following the naming convention below and then upload the document onto InPhase. This applies to all InPhase records, not just incidents.

The naming convention is: document name, purpose, version number – date uploaded – uploader initials.

For example:

  • PSII Report for divisional panel v3 – 17.09.26 – AGY
  • AAR Report for Trust panel v2 – 22.09.26 – JS
  • DoC Initial sent to patient v1 – 20.09.26 – TMA
