Today, Monday, 5th October is the start of our Perfect Discharge Week. Colleagues from across CHFT, Local Authorities, GPs and our wider health and care system will come together with one shared purpose; to ensure correct and safe patient discharges, at the right time, with the right support and without avoidable delay.

Trust executives, non-executive directors, Local Authority colleagues and wider system partners will be undertaking a series of 'go sees' across wards and departments at CRH and HRI to observe, identify opportunities for improvement and share good practice between teams.

A video has been created to help show colleagues how to run an effective board round in order to help with discharges. The video will be shown at the start of Perfect Discharge Week and will be used in further training once the week finishes as a way of helping newer colleagues understand the positive impact effective board rounds can have on length of stay.
