Following the successful offline trial of the Community Acquired Pressure Ulcer Triage Checklist, the checklist questions will be added into InPhase. By embedding this into the form, we aim to increase compliance and reduce the need for uploading additional documentation.
These questions will be visible for incidents of Community Acquired Pressure Damage where CHFT is the responsible organisation and the incident location is offsite/community.
This change goes live Thursday, 9th July 2026.
There will be no system downtime, and the new questions will not impact the incident reporting process. The checklist questions are for incident managers and investigators to be aware of and complete during incident review and investigation.
