Following feedback, a new Enhanced Care referrals process will be in Cerner EPR, via MTPL (multi-patient task list), from Wednesday, 1st July.

The team has also developed a preconfigured template as part of the new referral process to ensure that all interventions have been completed at ward level prior to the referral being made.

It will allow them to directly pull data to identify our highest and lowest referral areas, number of inappropriate referrals and any themes and trends, which will support us to work on quality improvement projects.
