The Cardiac Devices Team are introducing a new referral process, which will go live next Monday, 8th June.

The new referral form can be found on the CHFT Intranet Policies and Documents Library. You can limit your search results by typing 'cardiac devices' in the search field.

• Please complete as much detail as possible when referring a patient with a cardiac device for surgery or endoscopic procedure. This is to ensure a safe and efficient service for everyone involved.
• When booking a cardiac device patient for a procedure please book on a morning list or the first on the afternoon list, whilst avoiding certain days and times as specified on the form.

The form should be completed electronically before being sent to cardiac.devices@cht.nhs.uk, who will add the referral to EPR within two working days.

If the team need anything further, they will contact you on the information you have provided, before adding to EPR.

The aim is to phase out the paper referrals and be 100% electronic, so paper referrals will be rejected from Friday 26th of June.
