From this coming Wednesday, 1st April, referrals to Locala's Hospital at Home service will need to be made electronically.

To use the service, all referrers will need to open an account by signing up. Please set up your account as soon as possible to avoid any delays once the system goes live.

The referral link will be added to the Acute section, where other services, such as OPAT are currently located.

The Locala team will continue to update our capacity daily via Teams. Once the referral has been triaged, they will update the referring party in a timely manner to ensure this does not impact discharges. This will present as ‘accepted or declined’ on the response via email. They will also telephone initially until the system is bedded in.
