On Tuesday, 24th February there will be changes to the Learning from Patient Safety Events (LFPSE) Incident Form in InPhase. This is required to bring us up to date with national reporting guidelines.

There will be no system downtime and colleagues can continue to report incidents as usual.

There will be some new questions, and some existing questions will move.

A new Patient Safety Incident Reporting Framework (PSIRF) section will replace the current Strategic Executive Information System (StEIS).

Some questions will be duplicated. The incident and risk team are working to review this and, where possible, amend the form to reduce as much duplication as possible.
