NEW: High Consequence Infectious Diseases Policy
This new policy outlines our response to dealing with a suspected or confirmed case of a High Consequence Infectious Disease (HCID). It covers initial management and investigations of possible cases.
Revised to ensure alignment with AHP, Midwifery and Nursing National Frameworks. Revised section on monitoring and compliance, to include KPIs.
Discharge for Adults Group Policy
Reviewed to reflect changes recommended by the West Yorkshire ICB Adult Discharge/ Transfer of Care policy 2024. Recommendations from the Health Watch Report on discharge from hospital with reablement or intermediate care in place have also been implemented.
