Purpose T (Pressure Ulcer Risk Primary or Secondary evaluation Tool) is an evidence-based pressure ulcer risk assessment instrument that was developed using robust research methods. It identifies adults at risk of developing a pressure ulcer and supports nurse decision making to reduce that risk (primary prevention), but also identifies those with existing and previous pressure ulcers requiring secondary intervention and treatment.
As per the guidance on our Tissue Viability Pages, a reminder that PURPOSE T should be completed in the following instances:
Acute
- Within 6 hours of admission
- On change of clinical condition
- Weekly
- On ward transfer
- Following off Ward Medical/Surgical intervention – e.g. Theatre
Community
- First scheduled visit, or within 7 days of admission to caseload
- On change of care setting
- Readmission to caseload e.g. discharge from Hospital.
- At scheduled intervals according to risk assessment outcome/ Systmone care plan
- On change of clinical condition
