As part of the Kirklees Health and Care Partnership, Locala and Kirklees Council colleagues have been working with wider partners to identify how we can streamline our discharge pathways.
We have been specifically looking at our referral triage and allocation to capacity (beds and caseloads) processes between our collective teams.
The existing KILT (Kirklees Independent Living Team) referral form has been streamlined, and will be available from Wednesday, 12th November via the existing KILT referral portal.
What else is happening across the Partnership?
Locala and Kirklees Council teams will be working together to ensure for Reablement (REACH), Intermediate Care Beds (IMC) and Recovery Beds that the referral triage and allocation is co-ordinated at point of referral receipt.
For those pathways requiring social care worker or trusted assessment input (i.e. new packages of care, permanent care home placement) they will work to ensure that input is co-ordinated to support timely assessment and discharge on to the appropriate pathway.
The aim is to ultimately reduce the length of stay and improve outcomes for those patients needing support in the community after an acute episode of care.
