As part of the Quality and Value Programme there have been some updates to the Neighbourhood Therapy offer within Leeds Community Healthcare Trust. The aim is to provide referrers with a clear service offer and service eligibility. There are resources to support decision making when sending a referral, to ensure the patient is referred to the correct service offer the first time. Internal processes have also been streamlined to release more clinical time for therapists to deliver essential rehabilitation. This is vital to maintain patient flow and prevent admissions during the busy winter period and beyond.

  • Neighbourhood Therapy Service offer: Service description, contact details, service criteria and exclusions, and a supporting resource to signpost to alternative offers where exclusions exist. This is provided as both an online resource with expandable headers, or a full print version with hearers already expanded.
  • Neighbourhood therapy prioritisation guidance: Outlines information about how referrals are prioritised. Please refer to this guide when making priority requests, in particular requests for more urgent needs.
  • Neighbourhood Therapy referral process: Includes key results of a rapid improvement event which was held for therapy triage, actions taken, and communications about the minimum information we require when processing referrals. It also includes further guidance about what information helps make safe and quick decisions when prioritising referrals.
